If your insurance requires a referral order from PCP, contact your referring physician’s office for a referral order. Please have them fax the referral order at our office fax number- 813 588 4203. If there is no referral order prior to your visit, we will have to reschedule your appointment
1. All co-pays and deductibles are due at the time of your visit.
2. If you are not insured, we require payment at the time of service.
Please notify us at least 24 hours prior to your appointment time to cancel or change your appointment.
If you do not show up for your scheduled appointment and did not cancel 24 hours prior to appointment time, you will be subject to cancellation fees.
